What is the Cockcroft-Gault Formula?

The Cockcroft-Gault formula is a mathematical equation used to estimate creatinine clearance from serum creatinine. It is one of the most common methods used to approximate glomerular filtration rate (GFR) in clinical practice.

Although newer formulas like MDRD and CKD-EPI exist, Cockcroft-Gault is still widely used, especially for adjusting drug dosages for patients with renal impairment.

Why is it Important?

  • Provides a quick and inexpensive estimate of kidney function.
  • Essential for dosage adjustments of medications that are cleared by the kidneys to prevent toxicity.
  • Helps in the early detection and monitoring of chronic kidney disease (CKD).

Interpreting the Result (CKD Stages)

Stage 1: Normal or High GFR (≥ 90 mL/min): Kidney function is normal, but there may be other signs of kidney disease.
Stage 2: Mildly Decreased GFR (60-89 mL/min): A mild reduction in kidney function. Monitoring is recommended.
Stage 3a: Mild to Moderate Decrease (45-59 mL/min): A moderate reduction in kidney function.
Stage 3b: Moderate to Severe Decrease (30-44 mL/min): A significant reduction in kidney function. Medical management is important.
Stage 4: Severely Decreased GFR (15-29 mL/min): Severe kidney damage. Preparation for renal replacement therapy may be needed.
Stage 5: Kidney Failure (< 15 mL/min): End-stage renal disease. Dialysis or kidney transplant is necessary.
