What is the Srivastava Index?

The Srivastava Index is a simple formula calculated by dividing the Hemoglobin (Hb) level by the Mean Corpuscular Volume (MCV). It serves as a supplementary screening tool, similar to the Mentzer Index.

It is used to help distinguish between two common causes of microcytic anemia: iron deficiency anemia (IDA) and beta-thalassemia trait (BTT).

Why is it Used?

  • Offers an additional, simple mathematical clue in the differential diagnosis process.
  • Can be used alongside other indices like Mentzer and RDW for a more comprehensive initial assessment.
  • It requires no extra tests, as it uses values readily available from a standard Complete Blood Count (CBC).

Interpreting the Result (Reference Guide)

Index < 0.10: A value less than 0.10 is suggestive of Beta-Thalassemia Trait.
Index > 0.10: A value greater than 0.10 is suggestive of Iron Deficiency Anemia.

Important Clinical Note

This index, like other similar indices, is a screening tool and not a definitive diagnostic test. It has limitations and its accuracy can vary.

  • A definitive diagnosis always requires further testing.
  • Tests like serum ferritin, iron studies, and hemoglobin electrophoresis are essential.
  • The clinical context of the patient is paramount.
  • Consult a hematologist for a conclusive diagnosis and management plan.
