What is the PNFI?
The Pediatric Non-alcoholic Fatty Liver Disease (NAFLD) Fibrosis Index (PNFI) is a clinical scoring system specifically designed and validated for children.
It uses three simple parameters—age, waist circumference, and triglyceride level—to predict the likelihood of significant liver fibrosis in pediatric patients with NAFLD, helping to identify those who may need further evaluation, such as elastography or a liver biopsy.
Why is PNFI Important?
- Offers a simple, non-invasive, and low-cost method to screen for fibrosis in children.
- Helps pediatricians and gastroenterologists to stratify risk and guide clinical management.
- Can reduce the need for invasive procedures in low-risk children.

Management and Follow-up
The PNFI score is a guide for risk assessment. Management is focused on lifestyle interventions and addressing the root causes of NAFLD in children.
- Dietary Changes: Encourage a balanced diet, limiting sugary drinks and processed foods.
- Physical Activity: Promote at least 60 minutes of daily physical activity.
- Weight Management: Achieving and maintaining a healthy weight is the cornerstone of NAFLD treatment.
- Regular monitoring by a pediatric specialist is essential to track progress and liver health.
