Clinical Significance
The DiaRem score is a simple preoperative tool designed to estimate the likelihood of type 2 diabetes remission after metabolic surgery using only age, HbA1c, medication profile, and insulin use.
Although newer models exist, DiaRem remains useful because it is transparent, reproducible, and easy to calculate from routine clinical data.
Score Logic and Example
DiaRem uses four components: age, HbA1c, non-insulin diabetes medications, and insulin use. Scores range from 0 to 22, with lower scores indicating a higher chance of remission.
Age contributes 0 to 3 points, HbA1c contributes 0 to 6 points, specific non-insulin medication use contributes 0 or 3 points, and insulin contributes 0 or 10 points.
Example: age 45 years (1 point), HbA1c 7.8% (4 points), metformin only (0 points), no insulin (0 points) gives a total DiaRem score of 5.

DiaRem is best used as a counseling and risk-stratification aid before metabolic surgery. It should not be used alone to approve or deny surgery.
- Lower DiaRem scores generally indicate a greater chance of diabetes remission.
- The score was originally developed for preoperative remission prediction after RYGB and has later been externally evaluated in broader bariatric settings.
- Other models may perform as well or better in some cohorts, but DiaRem remains especially useful because it is simple and transparent.
- Interpret together with diabetes duration, procedure type, weight trajectory, and the overall clinical context.
