What is the GRACE score?
GRACE is a validated risk score used in acute coronary syndrome (ACS) to estimate short-term prognosis using readily available admission variables.
It supports clinical risk stratification and can help guide intensity of monitoring and management when used within local protocols.
Why use it?
- Standardizes bedside risk stratification in ACS.
- Helps identify high-risk patients who may benefit from closer monitoring and early invasive strategy (especially in NSTE-ACS).
- Facilitates communication and documentation of risk level.
Interpretation
- 🟢 Low risk: GRACE ≤ 108.
- 🟡 Intermediate risk: GRACE 109–140.
- 🔴 High risk: GRACE ≥ 141.
How to use the result
This tool supports clinical judgment and should be interpreted within local protocols and the patient context.
- Confirm correct units before interpreting the score (SBP mmHg/kPa; creatinine mg/dL or µmol/L).
- Use Killip class consistently at presentation.
- High-risk scores generally warrant closer monitoring and escalation per guidelines.
