What is the PRECISE-DAPT score?

PRECISE-DAPT is a validated 5-item score designed to predict out-of-hospital bleeding risk while a patient is treated with dual antiplatelet therapy after coronary stenting.

It uses age, creatinine clearance, baseline hemoglobin, baseline white blood cell count, and history of prior spontaneous bleeding.

It is mainly used to support decisions about DAPT duration when bleeding risk is high.

Why use it?

  • Standardizes bleeding risk estimation after PCI.
  • Helps identify patients who may benefit from shorter DAPT.
  • Supports individualized treatment decisions.

Interpretation (risk groups)

  • 🟢 Non-high bleeding risk: score ≤17.
  • 🟡 Moderate risk: score 18–24.
  • 🔴 High bleeding risk: score ≥25.

How to use the result

Use this score to support clinical judgment. DAPT duration decisions should also consider ischemic risk and PCI complexity.

  • Check units carefully before interpreting the result.
  • Use creatinine clearance rather than serum creatinine.
  • A high score supports caution with prolonged DAPT, but does not replace clinician judgment.
