What is the 4PEPS score?

4PEPS is a 4-level clinical pretest probability score for suspected pulmonary embolism.

It links the score to a diagnostic strategy: no test, D-dimer with optimized thresholds, or imaging.

Why use it?

  • It standardizes bedside assessment of suspected pulmonary embolism.
  • It may reduce unnecessary D-dimer or CT pulmonary angiography in appropriate patients.
  • It connects the score directly to the next diagnostic step.

Interpretation

  • Very low (< 0): rule out clinically without D-dimer.
  • Low (0–5): rule out if D-dimer < 1000 ng/mL.
  • Moderate (6–12): rule out if D-dimer below age-adjusted cutoff (or < 500 if age ≤ 50).
  • High (≥ 13): proceed to imaging (do not rely on D-dimer alone).

How to use the result

The 4PEPS score is a clinical decision-support tool used to estimate the pretest probability of pulmonary embolism in patients with suspected PE. It should not be used in isolation or as a substitute for full medical assessment. The result must always be interpreted together with the patient’s symptoms, vital signs, medical history, physical examination, and the clinician’s overall judgment. Local diagnostic pathways, D-dimer availability, and imaging protocols should also be taken into account before making a final decision.
